Waiver must be handed in once for the season. You can bring a hard copy to rink or email it to admin@hhskatingclub.com. Must be submitted prior to first skating session.
Health Screening Questionnaire: Health Screening Checklist - February 9.pdf
Screening must be handed in once for the Fall and once for the Winter. Must be completed at home 1-hr prior to each skating session. A verbal confirmation will be required at check-in.
